MILA Super Tier Plan Benefits Summary

The New MILA Super Tier Plan will be effective January 1, 2026, for workers who earned 2,000 or more hours during the October 1, 2024 – September 30, 2025, Contract Year. The Super Tier Plan will be the Premier Plan with the following enhancements:

A. Medical Plan

  • $0 Copays for:
    • Primary Care
    • Preventative Care
    • Specialist Care
    • Behavioral Health Care
    • Diagnostic Testing
    • Imaging
    • Urgent Care Visits
    • Emergency Room Visits
    • Rehabilitation
  • Hearing aid coverage at $2,500/ear every three years

B. Prescription Drug Plan

  • No Deductible and No Copays for in-network for:
    • Generic Drugs
    • Preferred Brand Name Drugs
    • Non-Preferred Brand Name Drugs

C. Dental Plan

  • Deductible: $0
  • Annual Maximum: $5,000
  • Orthodontic Maximum – Adult: $2,500
  • Orthodontic Maximum – Child: $5,000
  • Composite Fillings for Posterior Teeth

D. Vision Plan

  • $0 Copays for:
    • Eye Exams
    • Frames
    • Lenses
    • Contacts
  • Frequency:
    • Eye Exams: Every 12 months
    • Frames: Every 12 months
    • Contacts: Every 12 months
  • Allowances:
    • Frames: $200
    • Contacts: $200
    • Lasik: $2,000 per eye
